All subcontractors must submit key supporting documentation for review as part of our application process. As a guideline, the key items you must be prepared to submit include:
- Acknowledgement of our Drug & Alcohol Testing Policy & Procedures
- Acknowledgement of Safety Program policies & procedures
- Acknowledgement of our Railroad Title 32 Regulations
- Copy of your Employee Roster with supporting information
- Copy of an updated W9 in your company’s name
- Direct deposit information with a voided check and contact phone number
- Copy of Certificate of Insurance (COI) for respective job.
- Copy of your EMOD rate (if EMOD is in excess of 1.0, a copy of your OSHA 300 logs for the last 3 years must be submitted)
- Signed Master Service Agreement (MSA)
- Signed Rate Card for applicable projects (distributed by Project Management Office after subcontractor application acceptance)
- Acknowledgement of Damage Prevention & Arbitration Policy
- Acknowledgement of Quality Assurance Program Policy & Procedures
- Copy of legal entity documents (e.g., Articles of Incorporation, TIN Certification, Certification of Good Standing, etc.)
- Copy of Contractor’s License(s) for all states you’re licensed
